Interfaith CarePartners is a non-profit organization that works with partner congregations to assist their development of service programs that manifest each congregation's commitment to member care that serves both its own families and as an outreach program in the wider community. Interfaith CarePartners principals created the Care Team model of in-home support for families with special needs in 1985.
